Total G8 score is the sum of eight item scores (range 0–17; lower scores indicate greater frailty). Each item is scored as shown below; the total is compared with the standard cut-off of ≤14 to identify patients who should receive comprehensive geriatric assessment.

Total G8 = sum of all item scores. A total score ≤14 is considered an abnormal screen and usually triggers referral for comprehensive geriatric assessment.
