The revised Clinical Institute Withdrawal Assessment for Alcohol (CIWA-Ar) was developed in 1989 by shortening the original 15-item CIWA-A scale using data from 137 adults undergoing inpatient withdrawal at a Toronto addiction hospital. The 10-item total closely reproduced the full CIWA-A score (r²≈0.98) with high nurse inter-rater reliability, while items on seizures, quality of contact and vital signs were dropped because they added little predictive value.

Subsequent studies and guidelines have adopted CIWA-Ar as a standard symptom-severity scale for alcohol withdrawal in cooperative adults, particularly to support symptom-triggered benzodiazepine regimens and monitoring frequency in hospital, emergency and detox services. Many protocols classify scores <10 as mild or absent withdrawal, around 10–18 or 10–19 as moderate and ≥19–20 as severe, but exact cut-offs vary between services, so local thresholds should guide treatment decisions.

CIWA-Ar assesses current withdrawal symptom burden rather than diagnosing alcohol withdrawal or predicting complications on its own.
Scores should be interpreted alongside history, vital signs and risk factors such as previous withdrawal seizures or delirium, significant comorbid illness, pregnancy or polysubstance use.
Guidelines from ASAM, VA/DoD and Australian health departments recommend using CIWA-Ar only in alert, communicative patients; scores are unreliable in patients who are very sedated, delirious, language-barriered without an interpreter or intubated, where more objective tools are preferred.
Paediatric use is described only in small series and case reports, so CIWA-Ar should not be assumed to be validated for children or adolescents.
